Mail in Rebate Forms for 1800 Contacts

To redeem rebates for 1800 contacts, you need to go through 3 steps:

1. Make a qualifying purchase from 1-800 Contacts
2. Visit the rebates page to enter your order number and get your certificate(s)
3. Mail in the completed rebate certificate(s)

To get your rebate certificate, use the following instructions.

1. Head over to https://www.1800contacts.com/rebates
2. Enter your order number in order to access your unique rebate certificate(s). Print the rebate certificate and fill out the information fields. If your order qualifies for multiple rebates, print and complete each of the necessary forms. Remember, you can't use a rebate certificate more than once!

3. Mail the completed rebate certificate(s) via Mailform, to: 
   Rebate Processing Center
   P.O. Box 7777
   Sandy, UT 84091

Please allow 6-8 weeks for delivery of your rebate check. By using Mailform, you can complete this process entirely from your computer - no need to find a printer, go to the post office or anything else. Email team@mailform.io with questions, comments or suggestions.
